Updated Risk and Release Form

An updated University of Alaska Risk and Release form is now available at http://www.alaska.edu/swrisk/download/roac.pdf.

This form is only required for higher risk courses such as sports/recreation type activities, overnight field trips, camping and courses involving transportation. Departments who are unsure of the form's applicability to their course offerings should contact Trig at (907) 786-1351 or trig@uaa.alaska.edu for a risk analysis.

No other forms of this kind are authorized for use at UA for higher risk courses and the verbiage cannot be changed or modified without consultation with UA General Counsel. A special form is available for students under 18 years of age. After consulting with Trig, that form can be e-mailed to you. Form retention at the department level is three years after the course. For minors the retention is three years past students' 18th birthdays or three years after the course, whichever comes last. Please securely shred and recycle all paper that contains personally identifiable or other sensitive information.
