Obtain your biometric data (weight, height, blood pressure, waist measurement and total cholesterol) by attending one of the WIN for Alaska, Inc., Know Your Numbers screening events. Go to www.uahealthinaction.com for details and to view a schedule of biometric screenings in your area. Or, register online for your biometric screening appointment at http://uawinforalaska.timetrade.com.
Enter your biometric data into the Get the Point Tracking System to qualify for $50. Reach Tier 1 (at least 180 points and 2 consecutive months of tracking), to claim an additional $50. Or, mail in proof of your 2010 health club membership to qualify for $100. You may receive the JHCC 50/50 OR health club reimbursement, not both.
