June 2014: Additional on-campus biometric screenings scheduled

For employees in the Anchorage area, Human Resource Services has scheduled two days of biometric screenings. Appointments are available:

Thursday, June 19, 7:30 a.m.-1 p.m. (registration closes May 28) University Center, Room 114

Friday, June 20, 7:30 a.m.-1 p.m. (registration closes May 29) University Lake Building, Room 104

These will be the last on-site screening events before the June 30 deadline to qualify for the FY15 Wellness Rebate.

How do I sign up? 1. Go to my.blueprintforwellness.com. 2. Enter registration key: UOAonsite 3. Enter your username. You username is the first three letters of your first name, your full last name, two digits for your birth month and two digits for your birth day (e.g., George Clooney would enter geoclooney0506 as his username). 4. Enter your date of birth. 5. Select the Anchorage event and schedule your screening.

This is a fasting screening; please don't eat for 9-12 hours before your appointment. Please be sure to drink plenty of water and take your regular medication. Don't delay as the available appointments will fill up quickly and walk-in spots are limited.

Remember, the biometric screening is just one of the two activities you need to qualify for the $600 rebate. Be sure to also complete the Personal Health Assessment (PHA) at Healthyroads.com. Employees should use the Single Sign-On feature, using your UA username and password, with the link from the benefits website. Spouses (including employees who have opted out and are covered under their spouse's UA Choice plan) should go directly to Healthyroads.com to register and complete the PHA.
