Access to departmental labs, research labs, and other restricted spaces require approval by the Lab Director or Supervisor.  

Please click on the link below to submit the Laboratory Access Request Form.  You will need to know the following information to complete the form:

  • Name and email of the student/applicant (this is the individual requiring access).
  • Name and email of the Lab Director, supervisor, or faculty advisor.  A list of the lab directors and emails is available below.
  • Building and room numbers where access is required.
